Let begin with a quick introduction.  It is always nice to know who your are in class with.

What is your experience with Advanced care Planning in your practice?  Do you have a formalized practice?  Is it every provider for themselves?  Do you view it as a profit opportunity?  Do you see value in it as a practice activity?  Do you have a “best…[Read more]

I can’t count the number of times I was stuck with a sharps in my career.  Never did I report the incident.  I am sure I had reasons but they are hard to recall.  Needless to say I was lucky.

What are your concerns about accidental needle sticks at work?  Is it something you even think about?  Are you more or less likely to report the inciden…[Read more]

I am concerned about needle sticks and I do think about it. Sometimes I blame myself because I was distracted.

“Our hospital always says it is losing money. An impossibility in Maryland. The idea behind creating value and being paid foe value is to increase quality and decrease cost. This theoretically should attract more patients.”

Maryland is a special case.  In patient care is already paid under the preset budgeting system.  Here is the thing that m…[Read more]

      Dear Bryan:
You are so right! In Maryland it does not pay to be efficient. I think I told you I was the Deputy Secretary of Health under Governor Ehrlich. I am also very familiar with the HSCRC and the way rates are set in Maryland for hospitals. If a hospital is efficient; i.e., the cost to provide a service is less than what the HSCRC allows…[Read more]

One other item, how do you like the courses so far? I would be very interested in getting your opinion. Is there anything you would change or add? Is the course material easy to understand, applicable, and practical? Thanks for enrolling. If you have any colleagues who might have interest in the program, I would be pleased to speak with them.

My name is Dave Joyce and I am your class instructor.  I was an OB/GYN physician and practiced in the U.S. Army at Ft. Hood, Tx. and in a small group practice in Annapolis, Md.  I don’t practice anymore, but my wife does and I still retain what it was like to see patients every day and be on call.  Hopefully you will get a sense from the course th…[Read more]

I have actually spoken to more than a dozen men with ED and suing a needle is a non starter for them despite the fact that the results are so predictable.  A Lenis device would be an overwhelming positive addition for all of them.

With new tech the distinction between approved and unapproved is fuzzy. Physicians use “unapproved” devices and therapies all the time in an “off label” application. The device is approved by the FDA for injection. Is it approved for insulin? Who does the approval? Is it the payers, Medicare, FDA? What happens is that the market actually does the…[Read more]

I received this comment from Brooke Klatt:

“Also, it was a little bit confusing about FDA approval on Diabetes. In one of the slides it showed that FDA approval for Diabetes use was “*awaiting approval”, however insulin is one of the medications/indications for use. I very possibly could have missed something, which is why I found it con…[Read more]

Take a moment and share an experience with stem cell tissue products.  Do you have any advice your those starting out?  Any useful hints?  This promising technology has the potential to relieve a great deal of suffering.  Discussion among fellow clinicians can go a long way to moving the therapy forward and providing the greatest value for our patients.
