I have a question about PDSA case study. In the last VDO, You said we assume we know the causes when we create the survey. So how do you create a survey without being biased or wrong assumptions? Open question survey??

For dialysis clinic phosphorus problem, I agree with Rubeen’s process. For step 3, we always blame noncompliance as a first cause. It would be interesting to see from the patient side what they think and how they think we can help. I like the idea of theory Y. Everyone in the organization can engage and improve, strong team work. I might be wrong but theory X sounds like military order, at least from where I came from.
